Assurance Wellness Community Continuum Healthcare, LLC is a New Jersey–based, woman-owned home care agency dedicated to supporting individuals and families with compassionate, reliable, and community-centered care.
We provide non-medical home care services designed to help seniors, adults with disabilities, and individuals with chronic conditions remain safe, comfortable, and independent in their own homes. Our certified caregivers offer personalized assistance with daily activities, companionship, and household support, all under the supervision of a Registered Nurse in accordance with New Jersey Health Care Service Firm (HCSF) regulations.
Our mission is to strengthen the health and well-being of our community by delivering respectful, high-quality care that honors dignity, independence, and cultural sensitivity. We believe that care should be accessible, consistent, and tailored to each individual’s unique needs.
As we grow, our long-term vision includes expanding into mobile healthcare services and assisted living communities, creating a full continuum of care that supports individuals at every stage of life.
